Botox and dermal filler are the two most-asked-about treatments in any medical spa — and the two most commonly confused. They're not interchangeable. They address different concerns, work through different mechanisms, and last different amounts of time. Choosing well starts with knowing the difference.
What Botox (and Dysport, Xeomin) actually does
Botox is a neurotoxin. It temporarily relaxes the small muscles that create movement-related lines — most often forehead lines, frown lines between the brows ("11s"), and crow's feet around the eyes. It does not add volume. Results typically appear within a few days to two weeks and last around three to four months for most clients.
It's most often discussed for clients who notice lines that appear when they smile, frown, or raise their brows.
What dermal filler actually does
Dermal fillers — most commonly hyaluronic-acid based — restore or add volume. They're considered for areas like the cheeks, lips, under-eye hollows, jawline, and deeper folds around the mouth. Filler doesn't relax muscles; it sits in the tissue to restore structure and contour. Depending on the product and area, results can last several months to over a year.
It's most often discussed for clients who notice volume loss, flatness, or asymmetry — concerns that don't go away when the face is at rest.
When clients consider both
Many treatment plans use both, in small amounts, in different areas. Botox softens movement lines; filler restores structure. Used together by a careful provider, the goal is balance — not change. The most natural-looking results usually come from doing less, in the right places, over time.
