Our Commitment to Your Privacy

Maison Zolara LLC ("Maison Zolara") is committed to protecting the privacy of your Protected Health Information (PHI) as required by the Health Insurance Portability and Accountability Act (HIPAA). This Notice describes your legal rights and explains how we use and disclose your PHI. A copy is available at our Lansing studio upon request.

How We May Use and Disclose Your PHI

Treatment

We use your PHI to provide, coordinate, and manage your medical aesthetic treatments and related services. For example, your treatment history may be shared among our clinical staff (including our supervising physician) to ensure continuity and quality of care.

Payment

We may use and disclose your PHI to obtain payment for services rendered or to process billing. This may include submitting claims to your insurance provider (if applicable).

Health Care Operations

We may use and disclose your PHI for our internal operations, including quality improvement, staff training, compliance, and business management activities.

Other Permitted Disclosures

We may also disclose your PHI as required or permitted by law, including:

  • To report abuse, neglect, or domestic violence as required by law.
  • For public health activities (e.g., disease reporting).
  • In response to a court order, subpoena, or other lawful process.
  • To law enforcement under certain circumstances as permitted by law.
  • For national security or intelligence activities as required by law.

Uses Requiring Your Authorization

We will not use or disclose your PHI for purposes other than those described above without your written authorization, except as permitted or required by law. Uses requiring authorization include:

  • Marketing communications (beyond treatment reminders).
  • Sale of your PHI.
  • Use of psychotherapy notes (if applicable).
  • Most uses of your PHI not described in this notice.

You may revoke any authorization at any time in writing, except where we have already acted in reliance on it.

Our Duties

We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and notify you following a breach of unsecured PHI. We are required to abide by the terms of this Notice currently in effect.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already hold. The current Notice will be posted in our office and on our website. Any material changes will be communicated to you directly.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services (HHS). To file a complaint with us, contact our Privacy Officer using the information below. We will not retaliate against you for filing a complaint.
