Masseter Botox has a well-established safety profile when performed by trained clinicians using appropriate dosing. Understanding the realistic risks helps you make an informed decision.

Common, temporary side effects include mild swelling or bruising at the injection site, temporary jaw soreness, and a feeling of muscle fatigue in the first few days. These typically resolve within one to two weeks.

Chewing difficulty: Because the masseter is a chewing muscle, some patients notice mild difficulty with very tough foods — such as hard bread, raw carrots, or steak — in the weeks following treatment. This is expected, usually mild, and resolves as the body adapts. Avoiding excessively hard or chewy foods for the first two to four weeks is recommended.

Asymmetry risk: If the two masseter muscles are not of equal size — which is common — or if injection placement is not precise, mild asymmetry can result. Our licensed RN clinicians assess muscle symmetry before treatment and adjust dosing accordingly. Minor asymmetry can often be corrected at a follow-up appointment.

Smile and facial expression: In rare cases, if the neuromodulator diffuses beyond the masseter into adjacent muscles, it can temporarily affect the smile or facial expressions. This risk is minimized by precise injection technique and appropriate unit dosing. Choosing an experienced, anatomy-trained injector is the most important factor in avoiding this outcome.

Bone density — what the research actually says: Some patients have read about a study suggesting repeated masseter Botox injections may affect mandibular bone density. This finding, reported in a longitudinal study cited by the TMJ Association, involved patients receiving multiple injections and had significant limitations in sample size. Importantly, a 2025 double-blind, randomized, placebo-controlled study published in PubMed (NCBI/NIH) found no negative impact of onabotulinumtoxinA on mandibular bone density over 12 months in healthy adults with masseter muscle prominence. ↗ Appropriate dosing intervals, proper unit selection, and provider expertise are the key mitigating factors. The masseter is the large, powerful muscle that runs along the angle of the jaw on each side of the face. Its primary job is chewing, but in many people it becomes overdeveloped — a condition called masseteric hypertrophy — due to habitual teeth grinding (bruxism), jaw clenching, or simply genetics. An enlarged masseter creates a square or wide lower-face appearance and can cause chronic jaw pain, tension headaches, and tooth wear.

Botox Cosmetic (onabotulinumtoxinA) works by temporarily blocking the nerve signals that cause the masseter muscle to contract with full force. When injected into the belly of the masseter, the muscle gradually relaxes and, over several weeks, begins to reduce in volume. The result is a softer, more tapered jawline and — for patients with bruxism — meaningful relief from muscle-related tension.

The effects are temporary and reversible. As the neuromodulator naturally metabolizes over several months, the muscle gradually returns to its baseline activity. Most patients choose to maintain results with periodic touch-up sessions, which often require fewer units over time as the muscle adapts.

We will discuss realistic outcomes, unit estimates, and pricing — all confirmed before any treatment begins. Arrive with a clean face and avoid blood-thinning supplements (fish oil, vitamin E, aspirin if not medically required) for several days prior.

During Treatment

The appointment itself takes approximately 15–30 minutes. The masseter muscle is identified by palpation while you clench your jaw. Botox Cosmetic (onabotulinumtoxinA) is injected into the belly of each masseter using a fine needle. Most patients describe the sensation as a brief pinch. No anesthesia is required, though a topical numbing option is available on request. You can return to normal activities immediately after.

After Treatment

There is no downtime. Mild swelling or tenderness at the injection sites may occur and typically resolves within a few days. Avoid rubbing or massaging the injection sites for 24 hours, and avoid strenuous exercise for the remainder of the day. Results begin to appear at 2–4 weeks and reach their peak at 4–6 weeks. A follow-up check-in at 4–6 weeks allows us to assess results and address any asymmetry if needed.

Proper aftercare helps ensure the best possible results and minimizes the risk of complications. The guidelines below apply to the first 24–48 hours post-treatment and the weeks that follow.

• Keep your head upright for 4 hours after treatment
