What Causes Under-Eye Concerns

Understanding why your under-eye area looks the way it does is the single most important step toward choosing the right treatment. There are four primary causes — and each requires a different approach.

1. Structural (Volume Loss)

The tear-trough groove deepens when the fat pads beneath the orbital rim lose volume and the overlying skin loses elasticity. This creates the hollow, shadowed appearance often described as "sunken eyes" or "bags under eyes." No amount of energy-based treatment corrects structural volume loss. This cause is addressed with hyaluronic acid filler (specifically low-viscosity formulations) or PRF (platelet-rich fibrin) to restore lost volume. RF microneedling can improve surrounding skin quality but is not a standalone fix for structural hollowing.

2. Pigmentation (Melanin-Driven Darkness)

True pigmentary dark circles result from excess melanin production in the periorbital epidermis. For periorbital treatment specifically, the Matrix RF System is configured with a shallow penetration depth and lower energy settings to account for the significantly thinner skin of the under-eye zone. Step 1: Free Consultation

Your treatment begins with a complimentary consultation with one of our licensed RN clinicians. We photograph your under-eye area, assess your Fitzpatrick skin type, review your medical history (including medications, allergies, and recent procedures), and identify which of the four under-eye concern categories is your primary driver. Step 2: Pre-Treatment Preparation

In the two weeks before your session, avoid active retinoids, direct tanning, and sunburn in the treatment area. Arrive with clean, product-free skin. A topical numbing cream is applied 30–45 minutes before treatment begins to ensure your comfort during the session.

Step 3: The Treatment Session

Under-eye RF microneedling sessions typically run 30–45 minutes including numbing time. Your clinician applies the Matrix RF handpiece in a systematic pattern across the periorbital zone, using the periorbital-specific protocol settings. Most patients describe a warm pressure sensation during treatment. Eye shields are worn throughout. Immediately after, the skin appears mildly pink and may feel warm — this is expected.

Step 4: Immediate Aftercare

Mild redness, slight swelling, and a sensation similar to a mild sunburn are normal for the first 24–48 hours. Most patients return to normal activities the following day. The under-eye area may appear slightly more pronounced in the first 24 hours as mild swelling peaks.

Aftercare

Proper aftercare in the days following your under-eye treatment protects your results and supports healing in this delicate zone.

Do

  • Apply broad-spectrum SPF 30+ every morning, including overcast days — UV exposure after RF treatment can trigger pigmentation
  • Keep the treated area clean and gently moisturized
  • Use a cold compress (not ice directly) to relieve swelling in the first 24 hours if needed
  • Sleep with your head slightly elevated the first night to reduce periorbital swelling
  • Attend all scheduled follow-up sessions in your series
  • Contact us immediately if you notice unexpected blistering, spreading redness, or unusual swelling

Avoid

  • Retinoids (tretinoin, retinol), AHAs, and BHAs for at least one week post-treatment
  • Rubbing or pressing on the treated under-eye area — the skin is healing
  • Vigorous exercise and heat exposure (saunas, hot showers) for 48 hours
  • Eye makeup for 24 hours post-treatment
  • Sun exposure without sunscreen — especially critical in the periorbital zone
  • Picking or exfoliating the treated area during healing
